H.O.G. Chapter - Membership Sign-Up

**PLEASE READ**

The drive is on! Get your 2023 HOG Chapter Membership Enrollment and Release Form completed on/after January 1st and before January 31st to renew your HOG Chapter 4052 membership.

This is a HOG CHAPTER 4052 MEMBERSHIP REQUIREMENT.

As before, we don't charge or collect dues but we must get this form completed for you to be called a member of our Chapter!!  Membership has its privileges!  Get your newly signed form in by January 31st.

On February 1, 2023--only those completing the form (those with COMPLETED 2023 signed & SUBMITTED forms) will have access to our website and/or Facebook page.  As you might know, signing this document in the new calendar year is also a National H.O.G. annual requirement.

NOTE:  We will take receipt of digital sent enrollment forms to get you immediately signed up as a member---we MUST receive the original signed (wet-signature) copy of the form by January 31st or membership is null/void until we get the hardcopy/signed form.  Feel free to drop-off signed enrollment forms @ RHD (yellow HOG folder by Sales Tower).

Open Event: This event is open to Chapter members, National H.O.G. members and other guests as desired.
